Answers needed on transgender surgery pause in Wales, says health minister

Wales’ health minister has called a meeting with the NHS committee that ordered an immediate pause on gender-affirming surgeries, as questions mount over how long the suspension will last and what it means for patients on the waiting list.

Mabon ap Gwynfor, the Welsh health minister, is meeting members of the NHS Wales Joint Commissioning Committee (NWJCC) on Friday to discuss the decision that halted both new referrals and planned operations at the Welsh Gender Service. The committee directed Cardiff and Vale University Health Board to suspend the surgeries last week, citing concerns about the number of referrals and whether they were all appropriate.

The committee described the move as “precautionary and clinically-led”, pointing to data that showed “significantly higher rates of referral for gender affirming surgery… compared with English gender services” in 2025-26. An independent clinical review has been requested to examine the issue in detail.

For patients caught in the upheaval, the timing has been distressing. Cardiff and Vale University Health Board confirmed that some patients who had been awaiting gender-affirming surgery were already in hospital for their procedures when they were told of the suspension. The procedures — which encompass both masculinising and feminising operations, often involving reshaping of the genitalia — are irreversible.

A fast-moving dispute

The Welsh Gender Service provides referrals for patients living in Wales, but all NHS surgeries are carried out in England. The service has become a focal point for a debate that has been intensifying across the UK, where the rights and treatment of transgender people have become increasingly contested in public and political life.

First Minister Rhun ap Iorwerth has already pushed for a quick resolution. He said the committee that took the decision had done so “for the right reason” on service quality grounds, but he wanted the matter resolved swiftly.

The political reaction from other parties has been sharply divided. Reform UK and the Welsh Conservatives both welcomed the pause. Dan Thomas, Reform’s Welsh leader, said “sensible-minded people in Wales” would welcome the suspension and called for a comprehensive review of the service. Natasha Asghar, the Welsh Conservative spokesperson for health, said cancelling the surgeries was “absolutely the right thing to do” and had earlier called for a review.

Plaid Cymru, by contrast, expressed alarm. A letter signed by 26 of the party’s 28 backbench Senedd members was sent to Ian Green, chairman of the NWJCC, expressing “profound disappointment” with the decision. The letter, which emerged on Thursday, said the development had “caused distress, fear and a deep sense of uncertainty” among the trans community “at a time when the rights and dignity of trans people across the UK are increasingly under pressure”.

The Plaid members asked the committee to spell out what specifically had led to its concerns about the number of referrals, and when surgeries and referrals would resume.

Ap Gwynfor said he respected that the decision had been taken on the basis of clinical evidence, but he wanted answers. “I want to understand how long this review will take, what form will it take and to make sure that people out there know that services are there and that they can depend on the NHS in Wales to help them out in their hour of need,” he said.

Why it matters

The pause lands at a moment of heightened sensitivity around transgender healthcare in Britain. Gender-affirming surgery is a small but deeply significant part of the NHS pathway for transgender patients, and interruptions to planned care can be particularly distressing when the operations are irreversible. The fact that patients were already in hospital when the suspension was announced adds a sharp edge to the dispute.

The referral data that prompted the committee’s concern raises a broader question about demand and capacity in gender services — not only in Wales but across the UK, where waiting lists for gender identity care have become a persistent political and clinical issue. If the independent review confirms that referral rates in Wales are unusually high, the question becomes what drives that demand and how services should respond. If it finds the concerns were overstated, the question becomes how the committee reached its conclusion in the first place — and what that means for the patients caught in the middle.

What happens next

The immediate next step is the meeting between the health minister and the NWJCC on Friday. The committee has committed to an independent clinical review, but it has not said how long that review will take or what form it will take. Those are among the questions Ap Gwynfor has said he wants answered. Plaid Cymru has pressed for clear timescales. Patients and their advocates will be watching closely for any indication of when surgeries and referrals might resume.
